Transcripts and other records may be requested by mail or in person not by phone at the District Office, Monday – Friday, from 7:30 AM to 4:15 PM.
Processing fees: $2.00 per Transcript, $5.00 per Individual Education Plan (IEP) and $15.00 per Cumulative Record copy (currently enrolled students only). Only cash or money orders payable to Azusa Unified School District will be accepted. The Records Department does not accept personal checks, debit or credit cards.
To request records by mail: Please complete the Records Request Form and send the form, copy of I.D., and your payment to:

All requests will be processed within 3-5 business days. Records may be picked up at the District Office, faxed or mailed to the requesting party. Proof of ID must accompany all requests.
